Whiten for life

As a patient, you must attend your recall appointments every 6 months to obtain the free whitening tube, and if a visit is missed you will no longer be on the program and each tube required thereafter will cost $50 each.

If a patient misplaces or breaks their trays, or if the trays no longer fit due to changes in your dentition, it is not the responsibility of the clinic to provide new ones for free. New Trays may be purchased at full price.
